Thank you for your interest as a Vendor for the 2025 Polish Heritage Festival!
Please find the following forms attached:
- 2025 PHF Vendor Application
- Fire Permit Application (Vendors with propane/ LPG gas/ open flame)
- Temporary Food Establishment License (1 Day – $50)
- Vendors must bring their own tent, chairs, and table
- A tent/chairs/table will not be provided for vendors
- All vendors / helpers must have a blue wristband to enter

Reminder:
- Sale or consumption of any and all ALCOHOL is PROHIBITED.
- ALL Vendors Must Have Their Own Insurance.
- A copy of Certificate of Insurance must be submitted with the vendor application, naming Middlesex County Fair Association as additional insured.
- Food vendors must post items for sale and cost of each item at food stand.
- Polish Heritage Festival will provide only the space. Tents, electricity, etc. is the responsibility of the vendor. All Equipment, supplies, booth materials, and selling activity must be confined to your allocated space.
NO SMOKING IS PERMITTED IN ANY TENTS OR BUILDINGS BY ORDER OF THE EAST BRUNSWICK FIRE DEPARTMENT
